Transition IEPs

By state law, when a student with a developmental disability reaches age 13, it is time to begin school to work transition planning. From that age until graduation, you should be included in Individualized Education Plan (IEP) meetings to discuss post-high school dreams and plans or an Individualized Transition Plan (ITP). The ITP is an important part of the IEP.

Your high school transition coordinator should attend and assist in writing the IEP/ ITP beginning at age 13. Your regular and special education teachers, vocational education staff, your assigned DDDS manager/supports coordinator, your parents and you should attend every IEP meeting. During the IEP:

  • Confirm your year of graduation. Students with developmental disabilities are able to receive a free and appropriate education until age 21.
  • Find out whether you will receive a high school diploma or a certificate of attendance. There is a difference. Most employers request a copy of the high school diploma.
  • Be specific in writing goals for employment. You should tell the team what kind of work you enjoy and describe an ideal work setting and activities.
  • Discuss and plan for transportation from home to work. Public transportation training needs and public transportation services for those with disabilities should be included in the transition plan. Request several on-site, community-based job explorations provided through your school district, local intermediate unit, or an adult services provider agency. This will help assess your work preferences and abilities.
  • Request job-coaching services from local provider agencies to support you in work-study programs, usually offered during the final two years of high school.
  • Ask about job-coaching services for summer employment to help you maintain work skills. This may be considered under the “extended school year” option.
  • During the fall of your final year in high school, call your local Division of Vocational Rehabilitation (DVR) for assistance in preparing for and securing appropriate employment, community based work assessments, and job coaching services. Your DVR counselor will refer you to an adult services provider agency for many of these services.
  • Explore post-high school education and training options, such as community colleges and vocational schools, with your parents, the IEP team, your supports coordinator from DDDS, and a vocational counselor from the Division of Vocational Rehabilitation.  
    Call those colleges or vocational schools to ask about supports offered by their office of disability services. What non-credit courses do they offer to students with developmental disabilities? What documentation of disability do they require?
Request information about adult services provider agencies and their programs' ability to meet your needs and interests. Contact information for provider agencies should be added to your IEP's "Linkages Page."
